Stroke Recovery Support at Home in Kitchener-Waterloo Region

What is In-Home Stroke Recovery Support?

In-home stroke recovery support helps individuals manage everyday life at home while they participate in a rehabilitation program developed by their healthcare professionals. Promyse caregivers can assist with personal care, mobility, meals, daily routines, companionship and other activities identified in the individual’s care plan.

Promyse does not replace physiotherapists, occupational therapists, speech-language pathologists or other rehabilitation professionals. Instead, we collaborate with clients, families and community healthcare providers to support the individual between appointments and throughout their recovery at home.

We are happy to make a referral to a physiotherapist or occupational therapist that will come to your home, if this is a preference to going to their clinic.

Working With Your Stroke Rehabilitation Team

Recovery works best when everyone is working toward the same goals.

Stroke recovery may involve a team of community healthcare and rehabilitation professionals, including physiotherapists, occupational therapists, speech-language pathologists, physicians and nurses.

Promyse Home Care works alongside the healthcare and rehabilitation professionals already involved in your care. Depending on your needs, your community rehabilitation team may include a physician, nurse, physiotherapist, occupational therapist, speech-language pathologist or other healthcare professionals.

Support That Continues Beyond Rehabilitation

Promyse Home Care supports the Rehabilitaiton Plan at Home

Recovery after a stroke is different for every person. Some people need additional help primarily during the transition home, while others benefit from ongoing assistance.

As rehabilitation progresses, Promyse can adjust the level of home care support to reflect changing abilities, routines and goals. When formal rehabilitation services decrease or end, ongoing home care can continue to support safety, independence, social connection and quality of life.
