That desire for connection reflects decisions in 2025 and underpins almost every major transformation from closing the women’s health gap, increased recognition of the needs of neurodivergent adults with Autism and ADHD, and recognizing the importance for our senior loved ones to have the options of Aging in Place in their familiar homes. That connection in healthcare continues into the needs and wants in 2026.
Aging in Place Policy
Nine in ten Canadians over the age of 45 would prefer in-home care over moving to a long-term care facility. This number increased to 93% for those over the age of 65. For Canadians striving to age in place, having both support and reliable access to it is essential.
In 2025, provinces and national organizations took meaningful steps to move aging in place from aspiration to reality with the Aging in Place Challenge.
Most Canadians, particularly women, recognize the vital role Personal Support Workers (PSW) play in the lives of many, especially in facilitating the ability to age in place, a crucial factor for maintaining independence and dignity. Senior loved ones often juggle multiple health concerns and community needs.
PSWs provides direct, hands-on personal care services to clients, such as assistance with daily living activities (bathing, dressing, meal prep, light housekeeping), under the direction of the client or a regulated health professional. Their primary focus is the day-to-day supportive care established in their personal care plan.
Nurse Care Coordinators are your personal health Advocate and bring order to this complexity by aligning home care, rehabilitation, community programs, social supports and clinical follow-ups. They help ensure safe hospital-to-home transitions and monitor factors such as mobility, transportation and social isolation that influence long-term independence.
Nurse Care coordination can also help relieve pressure on family members who generally provide the navigation services for their loved ones. With the appropriate consents, nurse care coordination can become a family affair and not just a service provided directly to their elders.
