Promyse Home Care is proud to be part of the Family-Managed Home Care Program for families in the Kitchener, Waterloo and Cambridge area, to provide professional and compassionate care.
The Ontario Government, through Ontario Health atHome, runs the Family Managed Home Care program formerly known as Self directed Care.
Promyse Home Care understands the importance for families to live at home while receiving exceptional in-home care from professional providers. Through the Family Managed Home Care program, eligible clients receive funding to purchase home care services or employ care providers. This offers greater flexibility and choice as those approved for Family Managed Home Care choose their provider, determine how and when the care plan is delivered, and handle all related administrative tasks, including managing providers, funding, and reporting requirements.
Is Our Family Eligible?
Family Managed Home Care is designed for eligible clients in the following groups:
- Children with complex medical needs
- Eligible homeschooled children
- Clients in extraordinary circumstances, as assessed by Ontario Health atHome
- Patients in Extraordinary Circumstances
When Ontario Health atHome makes a decision on whether a client’s situation qualifies as an extraordinary circumstance, the following factors are considered:
- The nature, frequency, and intensity of care required
- Unique scheduling issues
- Language or communication barriers
- Cultural considerations
- Lack of Ontario Health atHome contracted service providers
- Rural or remote location of care

Working with Promyse Home Care
Once approved, an Ontaraio Health atHome Care Coordinator will regularly assess your loved one to determine the level of support needed on an ongoing basis. You’ll also be able to choose a professional in-home care agency like Promyse Home Care to help provide the necessary care.
Our Nurse Case Manager would be happy to assist you through the Family-Managed Care application process. Nurse Case Managers bridge these gaps by serving as the central point of contact, ensuring that all facets of a senior’s healthcare are seamlessly integrated and your loved ones receive the compassionate and professional home care they need, respecting their dignity.
